CLAIM FORMS

To commence making a claim on your policy, please follow this procedure:

1. Contact Business Insurance Specialists by phone on 1800 009 190 for instructions or

2. Look at your policy and discover the Insurer (Insurance Company).

3. Click on the appropriate Insurers (Insurance Company's) name at bottom of this procedure.

4. Click on the appropriate claim form for your Insurer (Insurance Company)

5. When the claim form is displayed print it out.

6. Fill in the claim form to the best of your ability.

7. Fax a copy of the completed claim form to Business Insurance Specialists on (07) 3102 9298 . Further information may be required, however this will commence actions. Business Insurance Specialists will contact to discuss further. Take all reasonable action to prevent any further damage or loss in the meantime.
